Membership Disclosure

Dental Implants & Cosmetics membership plans are not dental insurance, health insurance, or a substitute for insurance. Membership benefits are available only at Dental Implants & Cosmetics, located at 201 North University Drive, Suite 101, Plantation, Florida 33324, and only for services specifically included in the applicable written membership agreement. The plan does not pay or reimburse members or any third-party provider and may not be used with dental insurance, PPO discounts, third-party discount plans, promotions, coupons, financing offers, or any other discount unless approved in writing by the practice before treatment.

Membership plans are annual, individual, non-transferable, and have no cash value. Benefits are not retroactive and may not be assigned, transferred, exchanged, or refunded except as expressly provided in the written membership agreement or required by applicable law. Membership fees do not include services not specifically listed as included. Excluded services may include, without limitation, CBCT imaging, emergency visits, periodontal scaling and root planing, deep cleanings unless expressly included, implant treatment, surgical procedures, sedation, lab fees, medications, appliances, guards, retainers, missed appointment fees, specialist referrals, and services performed outside the practice.

All treatment is subject to clinical evaluation, patient eligibility, provider availability, and the professional judgment of the treating dentist. Discounts apply only to eligible services and may exclude certain procedures, products, lab-related charges, third-party costs, or specially priced services. Listed values, savings, and UCR comparisons are estimates for general informational purposes and are not guarantees of treatment need, treatment cost, clinical outcome, or insurance savings. Full membership terms, exclusions, cancellation rights, renewal terms, and payment obligations are governed by the written membership agreement signed at enrollment. Treatment discounts apply only to eligible treatment that is diagnosed, accepted, scheduled, and completed while the patient's membership is active, fully paid, and in good standing. Discounts do not apply retroactively, after expiration, after cancellation, or to treatment started before enrollment unless the practice approves otherwise in writing.

By enrolling, I understand that this membership automatically renews each year unless I cancel before the renewal date according to the membership agreement. I authorize Dental Implants & Cosmetics to charge the payment method on file for the then-current annual membership fee at renewal. I understand that membership benefits and treatment discounts apply only while my membership is active, fully paid, and in good standing.
